Although the two conditions are different, both can cause similar symptoms if a nearby nerve becomes irritated or compressed.
A bulging disc in the lower back does not always cause symptoms. Whether it causes pain often depends on its location, the structures around it, and whether a nerve is affected.
What are the symptoms of a bulging disc in the lower back?

This is one reason the size of a bulging disc on an MRI does not tell the whole story.

Sciatica is not the disc problem itself. It describes a pattern of nerve related symptoms that can travel from the lower back or buttock into the leg. The pain may feel sharp, burning, or electric. Some people also experience tingling, numbness, or weakness.
A bulging disc is one possible cause of this pattern, but it is not the only one. This distinction matters when deciding how to treat the problem.
What causes a bulging disc in the lower back?
The causes of a bulging disc often involve gradual changes rather than one major injury.
As we age, spinal discs naturally lose some water content and flexibility. This wear and tear can make a disc more likely to extend beyond its normal boundary.
Other factors may contribute, including repetitive bending or lifting, prolonged stress on the lower back, previous injury, and physically demanding activity.
Degenerative disc disease can also occur as discs change with age. The term does not mean that everyone with disc degeneration will have severe or chronic back pain.
Sometimes there is no single event that explains why a disc has changed.
How is a bulging disc in the lower back evaluated?
Evaluation starts with the symptoms rather than the scan.
We want to know where the pain begins, whether it travels into the leg, what activities make it worse, and whether numbness, tingling, or weakness is present.
A physical examination may also look at:
- Movement of the lower back
- Muscle strength
- Reflexes
- Sensation
- Walking
- Signs of nerve irritation
Imaging may be useful when clinically appropriate, especially when symptoms are severe, persistent, or associated with neurological changes.
An X-ray can show bones and spinal alignment, but it does not show the discs as clearly as an MRI. An MRI can provide more detail about the discs, spinal canal, and surrounding structures.
The goal is not simply to find a bulge. It is to determine whether the findings make sense when compared with the symptoms.
What treatment options are available for a lumbar bulging disc?
Treatment depends on the severity of the symptoms, nerve involvement, overall health, and how much the condition affects daily life.
Many people begin with conservative care rather than surgery.
Activity and gentle movement
Gentle activity, such as short walks, may help maintain mobility. Activities that worsen back or leg pain may need to be limited until symptoms improve.
Exercise and physical therapy
Exercise and physical therapy may help improve mobility and strengthen the muscles that support the lower back. Exercises should be selected based on individual symptoms, especially when a nerve is involved.
Medications and steroid injections
Pain medications may help manage symptoms, while steroid injections may be considered for persistent nerve related pain. These options should be discussed with an appropriate medical provider.
Non surgical spinal decompression
For selected patients with chronic disc related pain, spinal decompression may be a conservative option. It uses controlled traction to reduce stress on affected spinal structures. Treatment should be based on the patient’s symptoms and evaluation, not an MRI finding alone.
When does a bulging disc require surgery?
Most bulging discs can be treated without surgery. Surgery may be considered when severe symptoms do not improve with conservative treatment or when nerve compression causes significant or worsening weakness.
Seek urgent medical care if you develop:
- Loss of bladder or bowel control
- Numbness around the groin or inner thighs
- Significant or worsening leg weakness
These symptoms may indicate serious nerve compression and require prompt medical evaluation.
